• who make you feel comfortable and at home instantly.

During your first visit

• a thorough evaluation will be done to determine the source of your discomfort
• you will likely also receive some physical therapy that day.

Be sure to wear comfortable, loose-fitting clothing.

Please bring with you:

• Photo ID
• Insurance card
• Script/Referral from your healthcare practionaer
• Any X-Ray/MRI report/CD
• Medications list
• Completed new patient paperwork (if downloaded)
