Information you submit through this website

The appointment request form asks for your name, phone number, email address and preferred time. It does not ask for symptoms, diagnoses, insurance identifiers or other health information. If you choose to include health details in a free-text field, treat the submission as personal information and contact us by phone if you would rather not send it online.

Analytics and cookies

This build does not include third-party analytics, advertising pixels or tracking cookies. If analytics are added before launch, this policy will be updated to name the provider and the data collected.
